Good health is a team effort
Coordinating your care providers is one of the keys to healthy living. Follow these tips to make the most of your appointments.
Partnering with the health care providers who make up your care team can make a real difference in your well-being — both now and in the long run. Keep your primary care provider (PCP), any specialists you see, and your pharmacist informed about your health goals and medications.
Knowing what questions to ask at each visit — and keeping all members of your care team in the loop — can help you stay proactive about your health. Keep these two tips in mind:
Come prepared for health care visits with a few key questions that will keep everyone —including you — informed and on the same page about your well-being.
Be transparent with all of your providers and pharmacists about the details of your health, even if it’s a bit uncomfortable.
High-quality care, anytime
Your doctors want the best for your health — and so do we. At IBX, we support your care team with smart tools and details that help them spot care gaps, manage chronic conditions, and keep your health on track.
Our Population Health Specialists team up with your PCP to help you stay on top of important health checks, like cancer screenings and diabetes monitoring. They work with your provider to create a care plan that fits your needs, so nothing gets overlooked.
We bring doctors, specialists, and health systems together year-round to find better ways to deliver and coordinate care, so you can feel confident you’re getting the support you need, every step of the way.
Make the most of your next appointment with these simple steps
Three things to bring:
1. A list of your medications. Include your prescriptions and any over-the-counter medications you take. Be sure to add vitamins, minerals, and supplements too. If it’s easier, bring the bottles with you. This will help inform a conversation about any side effects you may have.
2. Your IBX member ID and a government-issued ID. Show your updated IBX Medicare Advantage card every time you visit your PCP, specialist, or pharmacy.
3. Your health journal or notes app. Before your visit, write down any symptoms, changes in your health, or questions you’ve had since your last visit. Bringing a list helps you remember everything you want to discuss. During or soon after the visit, record the key points you and your provider talked about.
Dr. Ramos says: No question or symptom is too awkward — your doctor has heard it all. Speak up to get the care you need.
Three questions to ask:
1. Are there any screenings, tests, or vaccines I should have this year?
Stay up to date on preventive care like cancer screenings, blood pressure checks, diabetes testing, and vaccines. Some may even be available during your visit.
2. How can I better manage my chronic conditions?
Ask for personalized advice on managing conditions such as diabetes, heart disease, arthritis, or depression.
3. Will a copy of my test results or notes from today’s visit be sent to others on my care team? Make sure your entire care team stays informed about your treatment plan and any updates from your appointment.
Knowing what to bring and what to ask can help you feel more confident for every appointment.
A little preparation goes a long way toward helping you get the most out of your visit — and the best care for your health.
