Providers pay close attention to your mental well-being, including your memory and mood. The goal is to keep your mind as healthy as your body. 

Medication 

Your care team works to make sure your medications are appropriate, effective, and safe for you. They regularly review all your prescriptions to avoid side effects and interactions that could harm your health. 

Living a full, healthy life 

Both new experiences and challenges often accompany getting older. The goal of  age-friendly health care is to ensure you live life as fully, healthily, and happily as possible, and that your family has support, respect, and understanding.
